WHO IS ELIGIBLE TO BECOME A PATIENT

The Open Arms Clinic serves Stephens County residents who are uninsured and do not have the means to purchase coverage. Residents who have or qualify for Medicaid are not eligible.

Please keep in mind that we do not provide mental health services or disability physicals. For mental health, we refer patients to Avita (https://www.avitapartners.org/); they are on a sliding scale. For disability, we will forward existing records upon request, but the exams for disability are provided by the disability agency you are seeing.

It's very important to complete the application as fully as applicable to your situation, as well as get the extra paperwork listed on pg. 2. It may seem excessive, but in order to get your needed medication once you are a patient, or obtain any referrals, they require those documents. It is easier for us and you to ask for these documents only once a year, instead of multiple times throughout the year.

When you have the application completed and the required documents, please bring them up to our office, in person, on any Tuesday (2pm-8pm) or Thursday (9am-5pm). You will have to wait for a bit (maybe 10 minutes or so), while we check over your application. From there, we will let you know if you might need anything else or if you're good to go. Once your application has been accepted, it should be about a week until you will be given an appointment.

Once eligible, residents can be seen at our clinic and receive their prescribed medications, free of charge.
